A leak by the vent. The vent had rusted through.

Opened up, the goose-neck had rotted with holes straight through the metal. Nothing on the surface suggested it — this is a component that fails from the inside.

New vent, new flashing, bedded in mastic, tiles reset and pointed around it.

Vents, goose-necks and flashings are the cheapest thing on a roof to fix and the most expensive to ignore.
